■Peroneal tendon yaralanmaları ayak bileğinde instabilite nedeni olabilir Peroneal tendon yaralanmalarına burkulmalar, subluk-sasyonlar, çıkık, peroneal tendonlardan biri veya her ikisinin yırtılması dahildir ve genellikle komplike olmayan burkulmalardaki gibi bileğin ters dönmesi sonucu oluşur. Sonrasında ayak bileği instabilitesi tedavi edilmeyen peroneal tendon yaralanmasına bağlı olarak gelişebilir. Peroneal tendonun subluksasyonu travmatik ayak bileği yaralanmalarının çok küçük bir kısmını oluşturur (°/o 0.3-0.5).
Peroneal tendon yaralanmaları sıklıkla yana hareket ve top kesme hareketi içeren, futbol, basketbol, Amerikan futbolu spor etkinlikleri sırasında ortaya çıkar ve genelde ters dönmüş ayağın dorsifleksiyonu sırasında peroneal kasların kasılması nedeniyle olur. Bu mekanizma superior peroneal retinakuluma hasar verir ve tekrarlayan subluksasyon veya çıkık ve sonrasında ayak bileği instabilitesine yol açar.36,38 Peroneal tendonlarm, özellikle de pe-roneus brevisin longitudinal yırtıklarında kronik subluksasyon da oluşabilir.
Peroneal tendon yaralanması olan hastalar incinme sırasında bir “çat”lama hissettiklerini bildirir. Ağrı tipik olarak lateral malleolün arkasındadır ve tekrarlayan subluksasyon spor etkinlikleri sırasında ayak bileğinin lateralin-de “çatırtı” şeklinde tarif edilir. Subakut ve kronik peroneal tendon yaralanması olan hastalarda, özellikle düzensiz zeminlerde instabilite sıktır.
Akut peroneal tendon yaralanmalarında ayak bileğinin posterolateralinde ağrı, şişlik ve güçsüzlük görülür; muayene bulguları peroneal tendonlar boyunca hassasiyet ve ödemdir. Peroneal brevis tendonunun subluksasyonu veya çıkığı ayağı plantar fleksi^on ve inversiyona getirip hastanın incinmiş ayak bileğim kuvvete karşı dorsifleksiyon ve eversiyona getirmesi istenerek belirlenebilir.
Düz grafiler genelde tek başına olan peroneal tendon yaralanmalarında normaldir. Fibulanın posterolateral kenarında kırık seyrek bulgudur, fakat peroneal retinakulum hasarını gösterir. Peroneal tendonlar ve stabilize eden retinakulum en iyi MRG ile görüntülenir, ancak küçük kınklar-dan şüpheleniliyorsa veya daha fazla değerlendirme de gerekiyorsa, BT kemik anatomisini de ayrıntılı olarak verir.
Subluksasyon veya çıkık cerrahi girişim gerektiğini gösterirPeroneal tendon incinmelerinde konservatif tedavi önerilir, fakat subluksasyon ve çıkıkta, özellikle de sorun tekrarlamışsa, cerrahi tedavi giderek daha fazla önerilmektedir. Konservatif tedavi yürüme botu veya desteği ile kısa süreli immobilizasyonun ardından, kuvveti ve hareketi arttırmaya yönelik fizik tedaviden oluşur. Subluksasyon ve çıkığın peroneal tendonlarm peroneal kanal içinde stabilize edilmesi şeklindeki cerrahi tedavisinin kalıcı stabilite ve düzelme sağladığı gösterilmiştir.
